Print out the electronic signature form, fill out, and fax to 573-335-7233 or 888-936-7234. This will be kept on file in the pharmacy.
- You have several choices on how to send your prescription to the pharmacy for dispensing.
All information may be typed directly on the form and saved. After completion, you can:
- Fax directly from your computer if you have that ability.
- Email the prescription directly by clicking on the "SEND NOW" button.
