Chronic Disease Management needs experience and expertise. Our nursing time combined has greater than 800 years of experience in managing patients with chronic diseases, including hepatitis C. The nursing team is employed directly by Special Design Healthcare and each committed to excellence in education, with highly individualized and comprehensive teaching. Our number one goal is to improve patient outcomes.
Our education can be provided one-on-one in the comfort of your home or in any clinical setting. Patients will be empowered with information about their specific disease state. Patients will be educated on medication administration, side effect management, dosing and potential interactions. The education is individualized and customized to meet specific need.
Patients can relax. Each patient will have access to a nurse and pharmacist 24/7. You will never feel alone. Monthly and weekly phone calls will be made to patient to schedule refills. The communication experience is unique and customized.
